2017 Youth Summer Outreach Program

Welcome to YSOP! We are looking forward to a great summer. Please take time to read this entire information before signing up your child. Space is limited. An email will be sent to you after the application deadline to inform you if your application is accepted.

Program Schedule

YSOP will be meeting every other Saturday from 11:00 AM to 2:00 PM.

Start date: June 10, 2017

End date: August 19, 2017

Drop-Off and Pick-Up

Children should not arrive earlier than 11:00 AM and must be picked up promptly at 2:00 PM.

Fees

The tuition fee for YSOP is FREE. Donations are accepted. A ten-dollar fee will be collected later for your child to get a YSOP t-shirt.

Participation in this activity may involve risk of injury. To my knowledge, my child has no health impairment that might interfere with or preclude any participation in the YSOP activities. As a parent or guardian, I am aware of these hazards and my child’s ability to participate. I hereby agree to release, discharge and hold harmless The Giving Network, its officers, directors, employees, and volunteers from any liabilities which may result from participating in this program. I understand that participation in any recreational activity involves risk. I further understand that The Giving Network does not provide accident/medical insurance for program participants. In addition, I give permission for my child(ren) to be treated by qualified medical personnel in the event that the named parent/guardian cannot be reached at the phone numbers provided. I also grant to The Giving Network, its representatives and employees the right to take photographs of myself and/or my child(ren) in connection with this program. I authorize The Giving Network, its assigns and transferees to copyright, use and publish the same in print and/or electronically. I agree that The Giving Network may use such photographs with or without names and for any lawful purpose.
